Attention all Parents of current 6th graders and Juniors
The Hancock Public Health Department is coming to the school on Wednesday, April 24, 2024 from 1-3 pm for a vaccine clinic for those current 6th graders that will be 7th graders and current Juniors that will be Seniors next year. All 7th graders are required to have Tdap and Meningococcal Vaccines and Juniors are required to have the Meningococcal vaccine prior to 2024-2025 school year.
Parents do not need to be present as long as the form is filled out, Initials beside the vaccine/vaccines, signed, dated and relationship to the student. The form can be found by clicking here and is also available in the office and in the School Nurse's clinic.
Please return forms ASAP, no later than April 22,2024 so we have an accurate count.
